GSK's Hibsago Receives FDA Approval: When Functional Cure Becomes a Commercial Decision

The U.S. Food and Drug Administration publicly announced approval of Hibsago (bepirovirsen), developed by GSK, for selected adults with chronic hepatitis B who are already receiving antiviral therapy.

The treatment is restricted to patients without cirrhosis, with suppressed HBV DNA and baseline HBsAg levels of 3,000 IU/mL or less, as determined by an FDA-authorized test.

Across two clinical trials, 24% of treated patients met the requirements to discontinue their existing antiviral treatment, while 19% maintained viral control and undetectable HBsAg 24 weeks after stopping all therapy.

The FDA also authorized Roche's Elecsys HBsAg II quant II RxDx companion diagnostic and required boxed warnings concerning serious liver reactions and thrombocytopenia. U.S. Food and Drug Administration

Why it matters strategically

Hibsago introduces a different therapeutic objective from conventional long-term hepatitis B suppression.

Existing nucleos(t)ide analogue therapies generally suppress viral replication but often require prolonged or lifelong administration.

Bepirovirsen offers some eligible patients the possibility of discontinuing treatment while maintaining viral control.

However, the clinical and commercial implications require careful separation.

A 19% functional cure rate at 24 weeks after treatment discontinuation does not establish permanent cure or equivalent long-term outcomes for every treated patient.

The treatment also requires 24 weeks of administration alongside existing antiviral therapy, followed by continued antiviral treatment for at least another 24 weeks before eligible patients may discontinue it.

The commercial value therefore depends on more than the reported functional cure percentage.
